المستخلص: انطلاقا من أهمية الدور الحيوي للتمويل الصحي مرتفع الأداء في بناء أنظمة صحية قوية وقادرة على الصمود في مواجهة الأزمات كما إن وجود هذه الأنظمة شرط ضروري لتحقيق التعافي الاقتصادي الشامل والمستدام كما برهنت على ذلك جائحة كوفيد- 19، علاوة على كونه استثمار في رأس المال البشري الذي يعد محددا رئيسيا للنمو والتنمية في الأجل الطويل وفقا للأدبيات الاقتصادية. تهدف الورقة إلي تتبع اتجاهات الإنفاق الصحي في مصر خلال الفترة (2015/2016- 2020/2021) من حيث قيمته ومصادر تمويله، يلي ذلك تقييم الإنفاق العام الصحي باستخدام مجموعة من المؤشرات التي تعكس الكفاية والكفاءة والعدالة مع مقارنته بمثيله في الدول ذات المستويات التنموية المتقاربة. انتهي التحليل إلى الإنفاق الصحي في مصر يواجه العديد من التحديات التي تؤثر سلبا على جاهزية النظام الصحي وقدرته على مواجهة الأزمات وبالتالي تعزيز النمو الاقتصادي في الأجل الطويل. ومن أبرز هذه التحديات: تفتت الإنفاق الصحي في مصر ما بين إنفاق عام متواضع وإنفاق مباشر ضخم مما يصعب حوكمة هذا الإنفاق وتعظيم الاستفادة منه، عدم وجود معايير معلنة لأسس توزيع الإنفاق العام الصحي على المحافظات، تقادم المؤشرات الصحية والافتقار للمؤشرات التي تعكس جودة الخدمات الصحية المقدمة وغيرها. في مواجهة التحديات السابقة يكون من الضروري مراجعة الإنفاق الصحي الحالي وتقييم كفاءته والعمل على إيجاد بدائل لزيادة الإنفاق العام الصحي وسرعة تنفيذ منظومة التأمين الصحي الشامل بعد مراجعتها مع كافة أصحاب المصلحة إلى جانب العمل على رفع كفاءة الإنفاق العام بحيث نقلل من عدم العدالة. وأخيرا، ضرورة تحفيز الاستثمارات الخاصة في القطاع الصحي مع وضع الضوابط التي تضمن عدم المغالاة في تكلفة الخدمات الصحية.

Based on the importance of the vital role of high-performance health financing in building strong health systems that are able to withstand crises, and the existence of these systems is a necessary condition for achieving comprehensive and sustainable economic recovery, as demonstrated by the Covid-19 pandemic, in addition to being an investment in human capital that It is a major determinant of long-term growth and development, according to economic literature. The paper aims to track health expenditure trends in Egypt during the period (2015/2016- 2020/2021) in terms of its value and funding sources, followed by an evaluation of public health spending using a set of indicators that reflect adequacy, efficiency and fairness, with comparison with that in countries with similar levels of development. . The analysis concluded that health spending in Egypt faces many challenges that negatively affect the readiness of the health system and its ability to face crises and thus promote economic growth in the long term. Among the most prominent of these challenges: the fragmentation of health spending in Egypt between modest public spending and huge direct spending, which makes it difficult to govern this spending and maximize its utilization, the lack of declared standards for the basis for the distribution of public health spending to the governorates, the obsolescence of health indicators and the lack of indicators that reflect the quality of health services provided and others. In the face of the previous challenges, it is necessary to review the current health spending, evaluate its efficiency, and work to find alternatives to increase public health spending and speed up the implementation of the comprehensive health insurance system after reviewing it with all stakeholders, in addition to working to raise the efficiency of public spending so that we reduce injustice. Finally, the need to stimulate private investments in the health sector while setting controls that ensure that the cost of health services is not overstated.
