520 |a The past few years have seen very active and productive research into the mechanisms that led to the structural MRI and the use of segmentation algorithms for the detection of the presence and degree of damage to mesial temporal structures. Despite the vast amount of literature on the MR-based volumetric measurements of the structures involved in the temporal lobe sclerosis, issues regarding including the septal nuclei remain an as-yet untapped approach. We have developed an approach for the septal nuclei segmentation in conjunction with the MR images. Multiplanar navigation software (DISPLAY) has been applied to correlate the volumetric measurements of the septal nuclei with the hippocampus in patients with temporal lobe epilepsy and to test the relationships between the volumes of the left versus right septal nuclei ipsilateral on the affected side. Segmentation of the hippocampus followed previous guidelines in three simultaneous planes, whereas the anatomical criteria for segmenting the septal nuclei were developed based on previous neuroanatomical knowledge, stereotaxic atlases and database search. The results for the hippocampus were consistent with the previous literature. The results showed that the protocol is applicable and markedly correlated with the hippocampus ipsilateral-to-seizure origin. We have demonstrated the feasibility of this approach that can be used with other EEG and neuropsychological tests for the post-surgical prediction of cognitive impairment. Indeed, it can be useful as a diagnostic tool for future pre-surgical evaluations.
