المستخلص: تهدف هذه الورقة البحثية إلى دراسة وتحليل مميزات محددات بلوغ الخدمات الصحية المتحكمة في قرار طالب العلاج في الجزائر، من خلال تناول حالة الخدمات الصحية الجوارية على مستوى المؤسسات العمومية للصحة الجوارية. تم إجراء الدراسة سنة 2012 وقد شملت 7 ولايات من الشرق الجزائري (أم البواقي، باتنة، تبسة، سكيكدة، قسنطينة، خنشلة وسوق أهراس)، كما تضمنت العينة 14 عيادة متعددة الخدمات من خلال اختيار هيكل صحي قاعدي في مقر الولاية والآخر خارجها، وعلى هذا الأساس بلغ عدد استمارات الاستبيان التي تمت الإجابة عليها 472 استمارة. وقد توصلت الدراسة إلى جملة من النتائج أهمها؛ أن هناك بعض الاختلاف في محددات المتحكمة في طلب خدمات الطب العام وطب المتخصص، أن المتحكم في قرار العلاج هو القرب المكاني لمؤسسات العلاج، بعض الأحكام المسبقة لطالب العلاج، بعض النزوات الذاتية... وليس الإمكانيات المالية التي يتمتع بها الفرد مثل ما تم التوصل إليه في جل الدراسات الأجنبية.

This research paper aims to study and analyze the characteristics of the determinants of access to health services that control the decision of the one seeking treatment in Algeria by exploring the state of the district health services at the level of public district health institutions. The study was conducted in 2012 and covered 7 provinces of eastern Algeria (Oum el Bouaghi, Batna, Tebessa, Skikda, Constantine, Khenchela and Souk Ahras). The sample also included 14 multi-service clinics by selecting a basic health structure at the province’s headquarters and another outside it. On this basis, 472 questionnaire forms were answered. The study reached a number of findings. The most significant of which is that there is some difference in the determinants of the demand for the services of general medicine and specialist medicine. That the control over the decision of treatment is the spatial proximity of treatment institutions, some pre-judgments made by those seeking treatment and some some subjective tendencies … In addition, the financial capabilities enjoyed by the individual are not the same as those which were found/concluded in most foreign studies.

Nous allons essayer à travers ce papier de cerner et d’analyser les déterminants de l’accessibilité aux soins qui influencent la décision des demandeurs de soins le cas de l’Algérie. Une étude a été réalisée durant l’année 2012 et qui a regroupé 7 wilayas de l’Est algérien (Oum El Bouaghi, Batna, Tébessa, Skikda, Constantine, Khenchla, Souk Ahras) dont 14 EPSP ; 2 EPSP dans chaque wilaya, l’un d’entre eux dans le chef lieu du wilaya, cette étude nous a permis de collecter 472 réponses des demandeurs de soins. Les résultats obtenus ont montré que les déterminants de l’accessibilité aux soins généraux ne sont pas les même par rapport aux soins spécialisés, les déterminants de l’accessibilité sont essentiellement la distance parcourue par le demandeur de soins, les préjugés sur la qualité des soins, quelques illusions subjectives…en contre partie les moyens financiers de l’individu influencent peu la décision de soins, ce dernier résultat qui ne s’accorde pas avec la majorité des études réalisées dans d’autres systèmes de santé.
