المستخلص: تمثل التغطية السكانية ببرامج التأمين الصحي إحدى وسائل الأمان الصحي للسكان، كما أنها تعد أداة مهمة لتجنب حدوث الإنفاق الكارثي على الصحة أو على الأقل خفض معدلات حدوثه، وتأتي أهمية دراسة التغطية التأمينية للسكان بالإسكندرية لرصد واقعها، وتقييم وضعها الحالي خاصة مع قرب تطبيق نظام التأمين الصحي الشامل بالمحافظة، وهدفت الدراسة من خلال الاعتماد على بيانات تعداد عام ٢٠١٧ وبيانات عينة الدراسة ٢٠٢١ إلى تحديد حجم التغطية السكانية بالتأمين الصحي بالإسكندرية وتباينها المكاني بين أقسام المحافظة، وتحديد خصائص التغطية التأمينية وأنواعها، فضلا عن تقييم مدى فاعليتها من خلال مجموعة من المؤشرات، وقد انتهت الدراسة إلى أن التغطية التأمينية تشمل نصف سكان الإسكندرية، وترتفع النسبة لتقترب من الثلثين بمدينة برج العرب الجديدة، في حين تنخفض عن النصف بمعظم أقسام وسط المحافظة، وترتفع نسبة التغطية التأمينية بالمحافظة بين الذكور أكثر من الإناث، وبين صغار وكبار السن عن متوسطي السن، وبين الفئات التعليمية الأعلى والمشتغلين خاصة في القطاعين الحكومي والخاص وبين السكان مرتفعي الدخل، وكانت النسبة الأكبر من التغطية التأمينية بالمحافظة تتبع الهيئة العامة للتأمين الصحي، وعكست مؤشرات فاعلية التغطية التأمينية التي اعتمدت عليها الدراسة وهي (نسبة التغطية التأمينية الحقيقية، مكان تلقي الخدمة، نوع الخدمة المقدمة، مستوى الرضا عن الخدمة) انخفاض فاعلية التغطية التأمينية بين أفراد العينة بالمحافظة. وأوصت الدراسة بضرورة وضع المشكلات التي تواجه السكان المؤمن عليهم في الاعتبار عند البدء في تطبيق التأمين الصحي الشامل بالمحافظة.

The Population coverage for health insurance programs represents one of the health security means for the population, and it is also an important tool to avoid the catastrophic spending on health. The importance of studying the insurance coverage of the population in Alexandria comes to monitor its reality and evaluate its current situation, especially with the imminent implementation of the comprehensive health insurance system in the governorate. The study aimed, by relying on the data of the 2017 census and the data of the study sample 2021, to determine the size of the population coverage for health insurance in Alexandria and its spatial variation between the districts of the governorate, and to determine the characteristics and types of insurance coverage, as well as to assess its effectiveness through a set of indicators. The study concluded that the insurance coverage includes half of Alexandria population, and the percentage rises to close to two-thirds in the new city of Burj Al Arab, while it drops to less than half in most of the central districts of the governorate. The insurance coverage rate in the governorate is higher among males than females, young and elderly people than middle-aged people, higher educational groups, workers especially in the public and private sectors, and among high-income populations. The largest percentage of insurance coverage in the governorate follows the General Authority for Health Insurance. The indicators of the effectiveness of the insurance coverage on which the study relied (the real insurance coverage percentage, the place of receiving the service, the type of service provided, the level of service satisfaction) reflected the low effectiveness of the insurance coverage in the governorate. The study recommended that the problems facing the insured population should be taken into consideration when applying the comprehensive health insurance in the governorate.
